For some reason my PTR 91 failed to cycle today when I was at the range. I could shoot the first round just fine but the bolt never came back and the spent casing remained in the chamber; I'd have to literally pull the cocking handle back every time I fired a round. I was using Federal 149gr so I doubt the ammo was the problem - I also encountered this error when using blanks. I didn't have this problem the last time I went to the range but between the two visits I replaced the bolt carrier with an official HK G3 bolt carrier and sold the one that came with the PTR (I'm still using the original bolt head, locking piece, spring, and firing pin - could these be causing the culprit?). If anyone could offer some insight to why I'm encountering this problem, I'd greatly appreciate it.

Mine is almost brand new; got it in January. I've fired somewhere between 100-150 rounds out of it.
I think I fired less than 10 rounds out of mine before I left the range in shame - it might as well have been a bolt action rifle, LOL!

I was advised it could be oil in the chamber but I haven't been able to make it back to the range to test fire since cleaning.
Could be oil in the chamber or it could be tar sealant or other detritus clogging the chamber flutes.
If the chamber is dry and the flutes clear of shmutz it will work.
I seriously doubt that the different bolt carrier changed a thing.

Sure could. Pray tell... why did you change out the carrier? Because it wasn't working? Having as much German stuff in the gun doesn't always work well as you've now found out.
You could now have any of a number of different problems. PTR had to use US made parts to comply with 922r (a certain number of parts in your rifle MUST be made in the USA, or you have an illegal gun). There could be clearance problems between the locking piece and the carrier. The carrier could be a bit worn, etc. Start by checking your bolt gap. There are PLENTY of threads discussing proper bolt gap so I won't go into it here.
