This is probably my biggest fear using reloads. Had to be ammo.

Reloads or not, why would the gun explode.
    Defective barrel and stuff?

Because Lady Physics has a way of chastising you for not calling her last weekend.
    Own: HK91, SR9, SP5k, G36k, HK416, P30L, P8, 2x P8A1, USP9
Future: Done, until HK comes out with something new & nifty. Might start building an FNH collection.

Because an inattentive reloader sometimes double charges or even under charges his cartridges. An under charged cartridge will at worst lodge a bullet in the barrel waiting for the subsequent round to blow the barrel up. At best you'll be lobbing projectiles........but hey, the recoil will be softer. A double charged cartridge will flat blow the gun up because the assembly is only designed for s certain amount of pressure. exceed that by enough and.....kaboom. Hasta lasagna, don't get any on ya. An over charged cartridge will blow primers and destroy brass but likely not destroy the gun because they are designed a bit over the average pressure for the cartridge. But don't plan on it....

If this tale is true, the shooter didn’t seem very surprised their gun exploded. I know if that had happened to me, I’d be taking pictures, calling people over as witnesses, etc.

Could the previous shot have been a squib round? Don’t know if that would make the next shot extra loud, but it definitely could have caused the spontaneous, traumatic disassembly.
