Filling out my form 1 for an HK 94 being made into an MP5 appearing gun (ie semi auto that looks like an MP5 with genuine HK parts), have a couple questions.

First box 4a obviously the manufacturer is Heckler & Koch the gun is marked as being made in W-Germany then it says Sterling, VA which is where HK USA is, so what do I put in 4a?

Second what is the overall length of an MP5 with an A2 stock on it? The gun has now an A2 stock, may be an A3 stock on it eventually, so what do I put in 4f for overall length? It will have a genuine 8.5" HK barrel.

4g, the serial number is 43-xxxxx whould I include the hyphen or not?

4i, what do people usually put for this?

13, name of maker, that is my name I presume?

Then below the signature of the CLEO, what is the "signature and title of delegated person"?

Finally how is it acceptable to affix my picture, glue, tape?

1. Sterling VA is sufficient
2. Accoding to World of HK http://www.hkpro.com/mp5.htm- BBL lenght is 8.85" and OAL is 26.77"
3. For consistency's sake just put the hyphen
4. Investment or other lawful purposes
5. Your name
6. Only is someone delegated by the CLEO signs off, I.E. a sherrif's deputy in charge of paperwork. Basically someone the CLEO assigns as his signature/autograph person /ubbthreads/images/graemlins/grin.gif.
7. My dealer taped mine with clear scotch tape just around the edge.
8. Hope all goes well. Post pics when the transformation is complete.

Thanks folks. More questions.

People talk about submitting the Form 1 with it printed on both sides of the page, the form is 4 pages, but information is only put on pages 1 and 2, do I need to submit all 4 pages (2 per side so 2 sheets of paper) or only pages 1 and 2 (1 sheet of paper doublesided).

Second where and what is this citizenship form I need to submit? This is for an SBR, thanks.

Here you go. All the forms you need. Also, you don't need to sent in the instructions p.3-p.4. Only two copies of the first two pages (on one piece of paper). /ubbthreads/images/graemlins/wink.gif

Thanks, on the citizenship form, who is the person certifying to citizenship? Someone other than I certifying that I'm a citizen?

The citizenship form is the retarded form of all forms! just fill it out and sign it yourself no witness no nothing General waste of time and tax payers money! /ubbthreads/images/graemlins/smirk.gif

I disagree, Commando. I submit to you that Form 1s and Form
4s should be modeled after the more lax requirements of
5320. That, when you think about it, is not retarded at all
and could save months of time in the application process.

...but more seriously, that 5320 takes what?...5 minutes to
fill out? Geez, would that it be all the Forms were so
stress free. Why is that so assinine? Would you prefer that
BATF ask for CLEO signoff...or cosigned by witness familiar
with the applicant, etc? Geez, this form is actually a no
brainer.

Sometimes I think that the folks who think this form is
useless also have little regard or integrity in their
own signature and honesty in declaring the information.
Conversely, when I sign off on something, I take it very
seriously and am swearing on my dad's grave the accuracy
of the information.

How's that for late evening blather? Actually, I'm still
at work - it's 7:15pm and I'm pissed as hell that I'm
still stuck here /ubbthreads/images/graemlins/smile.gif

I agree your signature and your honesty are everything but look how easy it is to forge this peice of paper ! honest people would never do it and that's my point ! it's like when you get a morgage and have to sign a document stating that it's you signing the document! /ubbthreads/images/graemlins/grin.gif

So I don't have to get someone else to sign my citizenship form for me, I just sign it myself?

Okay confused on where the forms should go after filled out. One adress I have been tols is in WV. One on this thread states DC and one one Atlanta Ga to a lock box. Can a veteran here whom has done this clarify please. Thanks Karl
